How do I create a working image mask?

I have tried to create an image mask. The image mask works until I need to update the rectangle behind it. The rectangle goes to the layer above the image mask. If I then update the image mask with any method. The transparent part of the image mask gets set to the background color, and you can't see the rectangle behind it.
